FTC "Red Flag" Rules - Tuesday, July 14, 2009

Effective August 1st, all Medical Offices in the U.S. must comply with new rules enacted by Congress and published through the F.T.C. regarding Identity Theft. As a result, all patient's must provide (at time of checkin) TWO forms of identification: either a valid driver's license or other government issues photo ID and your current Medical Insurance card. Also, your photo ID must list your current residential address. If it does not, please bring a recent utility bill listing your current address.

We thank you for your cooperation. We hope that these new rules will assist in the prevention and detection of Identity Theft.

We have implemented a new medical software system. Please make sure you bring your insurance cards with you to your office visit. As this is a new system, please allow yourselves a few extra minutes to fill out some additional paperwork before your appointment!

 
No Show Cancellation Fee - Thursday, November 13, 2008

We’re Sorry!

We have experienced an increasing number of “no-show’s” without a cancellation notice. 

With proper prior notice, we can give these appointment times to other patients who need to be seen. Please note that if you cannot keep your appointment we ask for at least twenty-four hours’ notice.

Your failure to keep your appointment without a minimum twenty-four notice may subject you to a $25.00 cancellation fee.
